• To display activities scheduled on a day, click that day in the calendar and the activities display on the right side of the window.

• To go to the next month, click the arrow after the name of the month at the top of the calendar.

Reservation and Request Forms

These forms are automatically submitted to the Church Office.

Room Reservation and Calendar Request Form

• To remove an existing activity from the calendar, please use this same form.

• Activities not using UUCM rooms should still be entered in the calendar with this request form.

Announcement Request Form

• Please submit to the church office by Wednesday evening (for the following Sunday).

Holiday Flowers Order Form

• Please submit to the church office by Wednesday, December 19th.

Sunday Flowers Order Form

• Please submit to the church office by Thursday at 9:00 a.m. (for the following Sunday).

Receipt Form for Tax Purposes

This form is for you to use to record cash you put in the plate on Sundays. Beginning in the 2007 tax year, the IRS will require evidence of all charitable contributions that you declare in your tax return. To access the form, click here, print it, record your donations, and have the Assistant Treasurer sign the form.
